“Anti-Infective Periodontal Therapy” A Non-Surgical Method for Treating Gum Disease
Periodontal diseases are now recognized as site-specific, communicable, bacterial infections that occur episodically. They therefore require constant monitoring.
The clinical manifestations of periodontal diseases are the result of a complex interaction between specific bacteria and the host’s immunological response. Most of the physical damage to the periodontium (supporting tissue of the teeth) is caused by the immunological response of the host’s immune system, not by the infecting bacteria or their by-products directly.
In this regard, periodontal diseases are analogous to other multi-microbial autoimmune diseases such as rheumatoid arthritis.
Anti-infective periodontal therapy employs our new understanding of the biological mechanisms of periodontal disease. Failure of treatment is due to the lack of elimination of the pathogenic bacteria or because the patient’s home care was insufficient to prevent re-infection.
We make use of microbiological assays, including bacterial cultures; phase contrast microscopy and DNA probes to reveal the microbial risk factors that precede and accompany periodontal disease.
Anti-infective therapy encompasses traditional professional debridement-mechanical therapy, but goes beyond by incorporating anti-microbial agents designed to reduce or eliminate “disease associated” microorganisms. Subgingival irrigation systems and the insertion of a variety of self releasing medicaments into the gum pockets are used to deliver local anti-microbial agents. Systemic antibiotics may also be used in combination with local therapy.
A promising new adjunctive addition now makes use of nutritional and plant-derived nutraceuticals taken orally to enhance the immune system. It is not an antibiotic.
